Mobile Care Team

Our 1800 employees work with close care and health in Uppsala County. We offer a wide range of services within primary care. Our patients meet us at Region Uppsala's own health centers or within habilitation, and we provide advice over the phone via 1177. Education, research, and development are important to us, as we adapt our services based on the needs of the residents. We already offer mobile teams, video visits, and smart e-services.

Your mission

The Mobile Care Team is part of the Close and Efficient Care, and the work differs from regular primary care; the care comes home to the patients. Our motto for our patients is “away is good, but home is best.” You have the opportunity for a varied job where no day is like the other and you can finally focus on patient-centered work.

In the mobile care team, you will make home visits together with the unit's nurses to individuals aged 17 and older. The main target group is patients over 65 years old, with acute medical problems of a somatic nature, who are assessed to be manageable at the primary care level. The operation also has follow-up visits from care neighbors. We are now also developing our working methods by trying to help identify suitable patients at the Academic Hospital for a quick follow-up.

Your qualifications

We are looking for you who are a specialist physician in general medicine, geriatrics, internal medicine, emergency medicine, or another suitable specialty. You should be confident in yourself and in your medical assessments. We work solution-oriented and see opportunities where others see obstacles, which we also hope you do. Swedish authorization as well as a very good ability to communicate in Swedish, both spoken and written, is a requirement. We place great emphasis on personal qualities. A valid driver's license and good driving experience are required.

Our operations

The Mobile Care Team and the Care Department belong to the management of Close Care and Health and are part of Region Uppsala. The Mobile Care Team is tasked with working with targeted primary care and developing the Mobile Care Team throughout Uppsala County. The mobile care teams are tasked with providing early interventions in the home, reducing avoidable hospital admissions and readmissions.
